I. My Health

1. Every day I experience extreme cravings, low energy, brain fog, lack of concentration, mood swings, headaches or sudden extreme hunger.

2. I am able to maintain healthy, balanced weight easily.

3. I have noticed changes in my health and I am concerned about becoming diabetic or I have been diagnosed as pre-diabetic.

4. My Doctor has indicated concern about issues such as: obesity, blood sugar level (pre-diabetes or diabetes), high blood pressure, high cholesterol, blood circulation issues, or kidney issues.

II. My Energy

1. My energy level is very high - all day, every day.

2. The food I eat sustains me and gives me consistently high energy throughout the day.

3. Stress is an issue for me and it constantly drains my energy.

4. I experience poor quality sleep consistently and it is affecting my energy level.

III. My Nutrition

1. I eat fresh vegetables and fruits several times a day (5 servings or more).

2. I often eat on the run, and my meals often consist of convenience foods, fast foods and refined foods.

3. I have a good knowledge of how foods affect my body's energy and blood sugar system: which foods give me energy and support healthy blood sugar, and which foods zap energy and damage blood sugar balance.

4. I experience continuous sugar cravings every day that sabotage my efforts and my intentions to eat fresh, healthy food.

IV. My Lifestyle Elements

1. I exercise at least 30 minutes a day 3 or more times per week.

2. I regularly sleep at least 7 restful hours per night and wake up feeling refreshed and energetic.

3. I am able to deal with stress easily and effectively.

4. I am able to understand and deal with my cravings easily and effectively.

V. My Diabetes Risk (1 = Yes - this applies to me, 10 = No - this does not apply to me)

1. My family has a history of Diabetes - brother, sister, mother, father, or grandparents.

2. I have a large amount of abdominal fat, indicated by a large waist circumference.

3. I am female and I have had either Gestational Diabetes (during pregnancy) OR I have been diagnosed with Polycystic Ovary Syndrome (PCOS).

4. I belong to one of the Ethnic groups that have a higher risk of Diabetes: African American, Hispanic/Latino, American or Canadian Aboriginal, Pacific Islander.

VI. My Commitment Level (1= not at all committed, 10 = highly committed!

1. I am ready to discover what I need to do to kick the sugar habit, gain energy, look good, feel great and stop pre-diabetes.

2. I am open to investing in the support and guidance I need to stop pre-diabetes and create a healthy lifestyle that lasts!

3. I am ready and willing to make healthy, sustainable changes in my nutrition and lifestyle so that I can reach my health goals and stop pre-diabetes.

4. I highly value the opportunity of a complementary Health Strategy Session and, if eligible, I will make it a priority book that session.
